Job Description:
Speech and Language Pathologists (SLP's) work with individuals who have a variety of speech related disorders.
Assist people who have problems understanding and producing language
Aid those with speech rhythm and fluency problems, such as stuttering
Assist those with cognitive communication impairments, such as attention, memory, and problem solving disorders
Help people who have oral motor problems causing eating and swallowing difficulties

State Issued License
Updated Resume
CPR Card (in person class only)
Physical from this year (This can be very basic, as simple as a quick prescription note from your doctor starting you are healthy and able to work or I have a form you can bring in)
TB from this year
TDAP (If you have one.)
Flu Vaccine (If it is flu season.)
MMR (Either 2 vaccinations or 1 positive Titer for Mumps, Rubella or Rubeola)
Hep B (If you have it)
Varicella (If you have it)
Two forms of ID (both ID's that you bring must have the same names. So if you were married it would have to show the same last name on both ID's.)
